Services Offered:


1. Chronic Care Management (CCM):
Personalized care plans for seniors with chronic conditions, leveraging advanced technologies and data-driven approaches.



2. Remote Patient Monitoring (RPM):

Responsibilities:



1. Compliance Oversight:


- Ensure all medical services comply with healthcare regulations and standards.


- Regularly update protocols to align with industry best practices and legal requirements.



2. Protocol Development and Implementation:


- Develop and implement medical protocols to enhance the efficiency and effectiveness of preventative care programs.



3. Provider Training and Development:


- Conduct training sessions for rendering providers to ensure understanding of Vitability Health's mission, protocols, and quality standards.



4. Chart Review and Quality Assurance:


- Oversee patient chart reviews to guarantee accurate documentation and adherence to established protocols.


- Implement quality assurance measures to enhance overall care quality.



5. Collaboration and Communication:


- Foster effective communication between medical staff, administrative personnel, and stakeholders.


- Collaborate with leadership to implement initiatives that enhance overall performance and impact.



Requirements:


- Medical degree from an accredited institution and active medical license.


- Leadership experience in healthcare, preferably in preventative care for seniors.


- Strong understanding of healthcare compliance, protocols, and quality assurance.


- Excellent communication and interpersonal skills.


- Ability to lead and inspire a multidisciplinary team.

Qualifications:



  • M.D., D.O.

  • Board Eligible / Certified

  • Internal Medicine background

  • Bilingual (English and Spanish)

  • Interest in Community Wellness and Care, Value Based Care, Technology, Elderly Care





Estimated Salary: $20 to $28 per hour based on qualifications.
