Position
Summary:
The Care Manager Supervisor is responsible for the oversight of the assigned care management unit.
The Supervisor works closely with Care Managers to ensure the overall provision and coordination of care management services.
The Care Manager Supervisor fully supports the development of.
care managers towards enhanced skills and job satisfaction as well as fully supporting methods and technology to support the quality care management and person-centered services for members.
Principal Duties and
Responsibilities:
oProvides support and direct supervision to Care Managers.
oMonitors assigned Care Managers' work activities with children and families through team meetings and regular individual meetings.
oManages, reviews, and monitors maintenance of member records to ensure both internal and external quality assurance standards and timely completion in accordance with agency and Health Home Policies.
oEnsures that new and existing care managers are fully trained in care management activities, current protocols and policies, and Federal, State and local laws, rules and regulations.
oTrains new Care Managers in their initial responsibilities including, CANS-NY assessment, Crisis Plans, Comprehensive Assessments, Progress Nates.
oTrains new Care Managers in the GSI system.
oTrains new Care Managers on how to conduct the HCBS/LOC.
oAssists Care Managers with the development of the Plan of Care.
oTrains new Supervisors, particularly for subcontracted agencies.
oUtilizes a person-centered approach to support members to ensure satisfaction with care management activities.
oReviews case notes and Life PlanStoo ensure quality and regulatory compliance.
oProvides supervisory oversight to ensure that plan goals are addressed in an appropriate timeframe.
oMonitors C!1fe managers' productivity.
oReviews manual, HIPPA laws, cultural sensitivities as part of the Care Management trainings.
Other Duties and
Responsibilities:
oProvides direct care management services in the absence of Care Management staff.
oAssists in locating and creating support networks in the community.
oWorks collaboratively with all teams throughout Agency programs, and other duties as assigned.
oPractice and promote Yeled's mission and values.
oAttends meetings within the Agency and outside the Agency as needed.
oOther job-related duties as may be necessary to carry out the responsibilities of this position.
Work Relationships and Scope:
Reports to the Director of HHCM.
Interacts regularly with Care Managers and other Program staff.
Performance Dimensions:
Maintain, and always seek to expand, knowledge of HHCM.
Keep up to date on changes to HHCM, and any other relevant program guidelines.
Must be sensitive to, and knowledgeable of the cultural needs of the community serviced.
Utilize knowledge of performance standards and program guidelines, combined with creativity and an understanding of the community being serviced in the implementation of quality programming.
Demonstrates the ability to support and model ethical practice when interacting with staff and families.
Must be accessible and responsive to the Program's needs.
Ability to perform and interpret needs analyses and translate the findings into actionable next steps.
A high level of confidentiality is expected of an employee in this position.
Working Conditions:
The Supervisor will work out of an office at his/her designated location.
Occasional kneeling, bending or reaching may be required.
Hours of work will generally be during normal site operating hours equaling 35 hours per week.
There may be occasional variation to the work hours due to special projects, deadlines or reviews.
Travel to Sites for meetings/trainings will be required.
Must hear and understand speech at normal levels.
Must speak so that others will be able to understand a normal in-person and telephone conversation.
Must be able to sit or stand for extended periods of time.
Must exert manual dexterity sufficient for keyboard and other office equipment operation.
