Patient Outreach Coordinator/Driver

The Patient Outreach Coordinator will be responsible for developing and growing professional relationships with substance abuse treatment providers, hospitals, medical professionals, and other key decision makers of prospective referral resources with the intent of creating long lasting referral relationships that allow clients the opportunity to utilize Urban Recovery as part of their treatment process, introduce Urban Recovery's services to the local community, providers, and agencies and to assist prospective clients in engaging with appropriate treatment services.
The Patient Outreach Coordinator will also be responsible for coordinating transportation needs of prospective or current clients.
Essential Duties
Responsibilities:
To provide marketing presentations to the community, providers and local agencies.
Work with local agencies to engage prospective clients in obtaining appropriate treatment services.
To provide transportation services to clients.
Inform the community and other local agencies about changes in the Elev8 Programs.
To maintain a New York State Driver's License in good standing.
To attend and be in compliance with all OASAS and Joint Commission Regulations as it relates to my job functionality.
Inform the executive team on a weekly basis on new provider linkage agreements.
Inform the executive team of needs in the marketing department.
To provide ideas on developing the Elev8 marketing brand.
To assist clinicians and the program administrators on transport services for discharge planning.
To assist in developing a Elev8 Centers Alumni Program.
Minimum
Qualifications:
High School Diploma At least 1 year of experience as a marketer working in the substance abuse treatment field in New York State.
Some computer skills are required.
